I didn't go to these places in Act I because I thought I could always go to them later. However now I'm in Act 2 and it looks like I can't go to these places anymore. Does that mean I'm permanently missing out on certain quests (like the Trial of the Maimed for Khelgar's vision quest)?

Is there any way to go back to these places once you're in Act II?

Unfortunately there is no way to go back to either of those places once you've started Act 2, so any quests associated with those areas are permanently cut off.

If you have an earlier save, it would definitely be worth reloading and completing those areas.
